Physicals
We are authorized by the US Citizenship and Immigration Services (USCIS) to conduct Immigration Physicals for Department of Homeland Security.
Please call our office to make an appointment for your Immigration Physical and bring the required documents listed below.
Immigration Physical includes the following:
- Basic Physical Examination and Evaluation using I-693 form
- Mandatory Blood work
- Verification of Vaccination Status.
- Follow protocol for missing vaccines or positive Tuberculosis test
In case of missing immunization records, vaccines, etc, you may need additional blood tests and/or vaccines that are required by USCIS for completion of medical exam. Some people may need a chest x-ray after screening test for TB. These services are available at our office and can be offered to you right away.
What you need to bring:
- Passport or valid government issued photo ID such as a driver’s license
- Immigration Physical Form I-693, pre-fill your information prior to the visit. (add link to I-693 form)
- Immunization Records of Proof of Vaccinations. (preferably translated in English if they are in a foreign language)
How long does an Immigration Exam take?
Immigration physical exam will take a minimum of two office visits. The first visit may take up to an hour. In order to prevent delay, pre-fill your section on I-693 Form and bring immunization records, preferably translated in english. Second visit may take thirty minutes or more, depending upon follow-up protocol required for completion of exam. Occasionally some people may need a third or additional visit(s).
Will I Need Vaccines?
Please bring all vaccination records to your scheduled appointment for the immigration physical so we can review your vaccination history with you to determine whether you have all the vaccinations required by USCIS. If you do not have appropriate immunization records, we can do a simple blood test or blood Titer to prove immunity. Blood Titer is a test that proves that a person has immunity or antibodies for a particular disease. In case you do not have the required vaccines, we can administer those for an additional charge at our office. A list of required vaccines is available here.
What happens upon completion of Physical Exam and After All R after all requirements are met?
After the immigration physical examination is completed, the Doctor will certify Form I-693 and place the same within a sealed envelope to be presented to the INS. DO NOT OPEN THE SEALED ENVELOPE. Submit the sealed envelope with your immigration application, follow instructions given to you by immigration authorities.

Fee for basic Physical Exam is $199 and due at the time of your first visit. It includes

Verification of Vaccination Status
We will not accept your Health Insurance as most plans do not cover Immigration Physicals. We can provide you paperwork, upon request, that you can submit to your insurance for reimbursement but we cannot guarantee that they will cover the cost.
